Patients Say

Positives

  • Rivastigmine transdermal patches are FDA-approved for cognitive impairment in Alzheimer’s and Parkinson’s dementia, with the patch route producing fewer GI side effects (nausea, vomiting) than the oral formulation — relevant for Long COVID patients with overlapping gastroparesis PubMed.

Negatives

  • Even among dementia patients prescribed Exelon, only about a quarter rate themselves moderately or markedly improved, suggesting most users see little visible cognitive change on the patch Lewy Body Dementia Association.
  • The patch frequently causes allergic contact dermatitis at the application site, and treatment is associated with nausea, vomiting, diarrhea, anorexia, and weight loss that can force discontinuation MedlinePlus.

Hurdles & Side Effects

  • Atypical cutaneous reactions to the rivastigmine patch are documented and require rotation of application sites; chronic use can also produce cholinergic effects (bradycardia, increased GI motility, urinary frequency) that compound autonomic symptoms in Long COVID PubMed.

  • Insurance authorization is essentially limited to Alzheimer’s and Parkinson’s dementia diagnoses; off-label Long COVID prescribing requires either a sympathetic neurologist or out-of-pocket payment of roughly $300-500/month Mayo Clinic.

Permanent remission of cognitive dysfunction and emotional dysregulation

Chronic Illnesses: Long COVID
Symptoms Treated: Brain Fog - Complete recovery

I was getting lost in familiar places, having panic attacks over small decisions like what to wear, struggling with word recall, unable to learn new skills, and emotionally volatile. I took rivastigmine transdermal 4.6mg for 6 months and it's almost completely gone now. I still get some flareups of brain fog associated with MCAS but I don't feel like I have dementia anymore, and my moods are much stabler. It didn't feel like it was doing anything until around month 5, but the results were astonishing and permanent.
